Profiles and Support Plans

Mosaic work with clients and families to develop a support plan so we can get to know how best to support you. As part of this process we compile a personal profile which is a document used by Mosaic staff to collect and record your support needs, likes and dislikes, key contacts, health and wellness, communication and behaviour, daily living skills, mobility and equipment, legal and ethical issues.
