If you wish to provide feedback to Mosaic, or if you are unhappy with the provision of supports and wish to make a complaint:

  • You can talk to a support worker, or someone else at Mosaic that you can trust

  • You can phone Mosaic on 6244 9444

  • You can fill out the feedback form available at Mosaic sites

  • You can ask for help from a family member, friend, advocate, or someone else you trust to make your complaint

  • You can fill out the feedback form available on the Mosaic website and Intranet

  • You can complete a confidential feedback/complaint form or send a letter by email to feedback@mosaictas.org.au with "confidential - Feedback" in the subject line, or by post to PO Box 4658 Bathurst St PO, Hobart 7000

Support for carers

  • Carers Tasmania - provide support to family and friend Carers across Tasmania including providing advice, counselling, support and respite

  • Care2Serve - the Tasmanian provider of the National Carer Gateway support services. They put Carers first, connecting them to practical advice and early intervention support services before they reach a crisis point.

  • Advocacy Tasmania - ‘Your Say’ advocacy is all about helping you be hear and understood. If you are unable to speak for yourself, or find it difficult, Advocay Tasmania can help

  • Speak Out Tasmania - supports people with disability and provides individual advocacy. They assist members to speak up and be heard.
